Job summary

20 hours per week, primarily weekends, with responsibilities across hospital and community settings. Ideal candidates hold RN licensure with at least 2 years of acute care experience and BLS certification.

Qualifications

• Graduate of an accredited nursing program with RN license required.
• BSN degree preferred.
• Minimum 2 years of acute care RN experience.
• Basic Life Support (BLS) certification required.

Responsibilities

• Introduce hospice care and services to patients and families.
• Assess goals for care and readiness for palliative approaches.
• Review medical records for disease progression and needs.
• Coordinate admissions and hospice levels of care with providers.
• Arrange equipment, medications, and supplies for home hospice.
• Educate referral sources and hospital staff about hospice care.
• Support patient and family through anticipatory grief and decision making.
