Wellstar Health System is seeking a Part-time RN Hospice Liaison to join the Kennestone campus team. This role involves educating patients and families about hospice, coordinating admissions, and collaborating with physicians and care teams. The position requires an active RN license, BLS, and at least 2 years of acute care experience.

Qualifications

• Graduate of an accredited/approved school of nursing.
• Baccalaureate degree in nursing preferred.
• Active RN license (Single State) required; Multi-state Compact preferred.
• Basic Life Support (BLS) required; BLS Instructor/Provisional preferred.
• Georgia driver's license and private vehicle insurance required.
• Minimum 2 years of acute care RN experience.

Responsibilities

• Introduces hospice care and services to the Patient and family.
• Evaluates goals for care and readiness to accept a palliative approach.
• Reviews the Medical record for disease progression and decline.
• Assesses patient condition and identifies needs.
• Discusses advance directives related to hydration, nutrition, antibiotics, and code status.
• Collaborates with physicians/NPs on hospice assessment and referrals.
• Obtains orders to admit to hospice and coordinates care levels with the physician.
• Obtains verbal certification from the attending physician and admits the patient.
• Coordinates hospice admission with intake and makes discharge arrangements.
• Orders medications, equipment, and supplies to ensure timely delivery.
• Educates referral sources and hospital staff about hospice care models.
• Supports patient and family in anticipatory grief and decision making.
• Respects religious and cultural differences in end-of-life care and finances.
