Job summary

Wellstar Health System is seeking a part-time RN hospice liaison for 20 hours per week, primarily weekends at the Kennestone campus. You will educate patients and families about hospice services and facilitate referrals and admissions, coordinating with physicians and hospital staff to ensure smooth transitions into home, IPU, SNF, or other settings.

The role requires active RN licensure in Georgia, BLS certification, and 2+ years of acute care experience, with strong communication and clinical

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Licensed Registered Nurse in Georgia; BLS certification required.
• Minimum 2 years of acute care RN experience.
• Ability to read, write, and speak English; strong tech skills.

Responsibilities

• Introduce hospice care and services to patients and families.
• Assess goals for care and readiness for palliative approach.
• Review medical records for disease progression and decline.
• Coordinate admissions and arrange discharge from hospital.
• Order medications, equipment and supplies for Hospice care.
• Educate referral sources and hospital staff about hospice care.
• Support anticipatory grief and involve family in care planning.
• Respect religious and cultural differences in end-of-life decisions.
