Job summary

Wellstar Health System in Georgia seeks a part-time RN Hospice Liaison. 20 hours per week, primarily weekends at Kennestone campus. You will educate patients and families about hospice options, assess eligibility, and coordinate referrals and admissions.

You will work with physicians, hospital teams, and referral sources to ensure smooth transitions to hospice services and high-quality end-of-life care, guided by Wellstar practice standards.

Qualifications

• Graduate of an accredited nursing program with RN license.
• Bachelor's degree in nursing preferred.
• Minimum 2 years of acute care RN experience.
• Strong written and verbal communication skills.
• Must have a Georgia driver's license and reliable transportation.

Responsibilities

• Introduces hospice care and services to the patient and family.
• Evaluates goals for care and readiness for a palliative approach.
• Reviews medical records for disease progression and needs.
• Assesses current condition and identifies care requirements.
• Coordinates admission with physicians, hospice intake, and arrange transport as needed.
• Orders medication, equipment, and supplies to arrive before patient return home.
• Educates referral sources and hospital staff on hospice care models the holistic approach.
• Supports patient and family in anticipatory grief and decision making.
