Job summary

The nurse will lead interprofessional care planning, supervise routine nursing tasks, and ensure documentation and adherence to agency standards while maintaining patient safety and comfort.

Qualifications

• Active Registered Nurse license in state of practice.
• Basic Life Support (BLS) certification for Healthcare Providers.
• Valid driver license and access to reliable transportation.

Responsibilities

• Identify and coordinate patient/family care to support terminally ill patients in hospice settings.
• Educate patients and families on disease processes, trajectory, and end-of-life care.
• Lead IDG care planning and coordinate services with interdisciplinary team to implement the plan of care.
