Job summary

Aurora Health Care is seeking a Registered Nurse to manage caseloads in specialty areas, perform initial assessments, and establish comprehensive care plans aligned with physician orders. You will deliver direct patient care, adjust nursing strategies, and document accurately to support ongoing treatment and outcomes.

Responsibilities include communicating with the medical team, participating in discharge planning, and contributing to quality improvement initiatives.

• Proficiency in clinical skills with sound judgment and decision-making.
• Ability to assess patient data and interpret information to identify needs.

• Must possess a valid Driver License and a registered vehicle with insurance.

Responsibilities

• Assumes responsibility for caseloads in specialty areas.
• Conducts initial assessment to obtain comprehensive physical, social and environmental data.
• Establishes the plan of care per physician orders and patient needs.
• Provides direct patient care and adjusts nursing care as needed; evaluates outcomes.
• Performs ongoing assessment of patient response and updates care plan.
• Documents visit assessments, OASIS, 485 and verbal orders accurately.
• Communicates with physicians to confirm/update plan of care and keeps team informed.
• Facilitates discharge planning and continuity of care after discharge.
• Participates in chart audits and quality improvement activities.
• Maintains accurate clinical and patient records per guidelines.
