Job summary

Aurora Health Care is seeking a Hospice Registered Nurse to provide compassionate end-of-life care in patient homes and community settings. You will perform comprehensive nursing assessments, triage, and interventions during after-hours shifts to support hospice goals of care.

Qualifications

• Completion of an accredited or approved nursing program.
• Active RN license in the state of practice.
• Valid driver’s license.
• BLS certification — American Heart Association.
• Typically one year of nursing experience in hospice or related clinical settings.
• Strong hospice philosophy and end-of-life care understanding.
• Excellent communication and crisis-management abilities.
• Ability to travel to patient homes and community settings.

Responsibilities

• Provide comprehensive nursing assessment, triage, and intervention for hospice patients during after-hours shifts.
• Respond to patient and caregiver calls to assess symptoms, educate, and determine appropriate interventions.
• Conduct in-person visits to manage acute symptom changes and ensure patient comfort and safety.
• Manage complex end-of-life symptoms, including pain and dyspnea.
• Initiate and carry out physician orders and standing hospice protocols within scope of practice.
• Document all patient encounters and communications in the EMR with timely handoffs.
• Collaborate with physicians, AP providers, and interdisciplinary team members to coordinate care.
• Adhere to hospice policies, regulatory requirements, and safety initiatives.
