Qualifications

Responsibilities

• Identify and coordinate patient/family care in hospice settings.
• Educate patients and families on disease processes and end-of-life care.
• Lead the Interdisciplinary Care Group (IDG) and develop the Plan of Care.
• Administer medications and treatments as prescribed.
• Document in the electronic health record per standards.
• Discharge planning and coordination with community resources.
