Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other healthcare professionals
• Assess patient eligibility for hospice by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families
• Educate patients and families on hospice care, how it works, and why it matters with clarity and compassion
• Complete necessary documentation and coordinate intake and admission to meet legal, clinical, and payer requirements
• Serve as a liaison between referral sources and the hospice care team for seamless handoffs and rapid responses
• Make daily visits to hospitals and facilities to stay visible with discharge planners, case managers, and physicians
• Build and maintain relationships with referral partners and identify outreach opportunities to increase hospice awareness
