Job summary

Gentiva Hospice is seeking an experienced Hospice Clinical Liaison to optimize intake and referrals, coordinate admissions, and build strong partnerships with hospitals, physicians, and discharge planners. You will travel to facilities to ensure timely, compassionate care and educate families about hospice services.

The role requires an RN license with hospice experience and a track record of cross-functional collaboration in a fast-paced environment.

Qualifications

• Licensed RN with 3–5 years hospice experience required.
• BSN with 5+ years hospice experience preferred.
• Minimum of 3 years hospice experience required.
• Active RN license in the employment state and CPR certification.
• Valid driver's license and auto insurance.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals.
• Assess patient eligibility for hospice by reviewing records and consulting physicians.
• Educate patients and families about hospice care, processes, and benefits.
• Coordinate intake and admissions to meet legal and payer requirements.
• Serve as liaison between referral sources and care team for seamless handoffs.
• Make daily visits to hospitals and facilities to stay connected with discharge planners and physicians.
• Develop and maintain relationships with referral partners and identify outreach opportunities.
