Job summary

Gentiva is seeking an experienced Hospice Clinical Liaison to ensure efficient intake and admissions, build relationships with referral sources, and support community awareness of hospice services. You will visit hospitals and facilities, educate families about hospice care, coordinate documentation, and collaborate with the care team to deliver compassionate, compliant service.

Qualifications

• RN license to practice nursing in the state of employment.
• 3–5 years of hospice experience required.
• BSN with 5+ years hospice experience preferred.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other healthcare professionals.
• Assess patient eligibility for hospice services by reviewing records and communicating with physicians and families.
• Educate patients and families on hospice care, how it works, and why it matters.
• Complete necessary documentation and coordinate intake and admission processes.
• Serve as a liaison between referral sources and the hospice care team for seamless handoffs.
• Make daily visits to hospitals and facilities to maintain visibility with discharge planners and providers.
• Build and maintain relationships with referral partners and identify outreach opportunities.
