Job summary

Gentiva Hospice is seeking an experienced Hospice Clinical Liaison to streamline intake/referral processes, coordinate admissions, and educate patients and families about hospice care. You will build strong relationships with hospitals, physicians, case managers, and discharge planners to ensure timely and compassionate care.

The role requires RN licensure with three to five years hospice experience; BSN is preferred.

Qualifications

• Licensed registered nurse with hospice experience is required.
• BSN with hospice experience preferred is advantageous.
• Minimum three years of hospice experience.

Responsibilities

• Receive and respond to referrals from hospitals and other sources.
• Assess patient eligibility for hospice services.
• Educate patients and families on hospice care.
• Complete intake documentation and coordinate admissions.
• Serve as liaison between referral sources and hospice team.
• Make daily visits to hospitals and facilities.
• Build relationships with referral partners and identify outreach opportunities.
