Job summary

Gentiva Hospice is seeking an experienced Hospice Clinical Liaison to streamline intake and referral processes, coordinate admissions, and strengthen relationships with hospitals and referral sources. The role requires strong clinical judgment, compassionate communication, and the ability to educate families about hospice services.

Qualified candidates hold an active RN license, with BSN preferred, and at least three to five years of hospice experience.

Qualifications

• Licensed RN in the state of employment.
• Three to five years of hospice experience required.
• Minimum of three years hospice experience and related roles preferred (discharge planner, case manager, educator).

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other healthcare professionals.
• Assess patient eligibility for hospice by reviewing medical records and communicating with physicians and families.
• Educate patients and families on hospice care, its scope, and benefits with clarity and compassion.
• Complete documentation and coordinate intake/admission, ensuring legal, clinical, and payer requirements are met.
• Serve as liaison between referral sources and the hospice care team for seamless handoffs.
