Job summary

Gentiva is seeking an experienced Hospice Clinical Liaison to streamline intake and referrals, coordinate admissions, and strengthen relationships with hospitals and referral sources. The role emphasizes compassionate patient-family education and efficient care handoffs.

The ideal candidate holds an RN license with hospice experience and is prepared to conduct daily visits and collaborate with multidisciplinary teams to ensure timely, high-quality hospice services.

Qualifications

• Licensed nurse with hospice experience is required.
• Minimum of three years hospice experience is required.
• Current RN license in the state of employment.
• CPR certification is required.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals.
• Assess patient eligibility for hospice by reviewing records and communicating with families and physicians.
• Educate patients and families about hospice care, processes, and benefits.
• Complete documentation and coordinate intake/admission to meet legal and payer requirements.
• Serve as liaison between referral sources and the hospice care team for seamless handoffs.
• Make daily visits to hospitals and facilities to maintain visibility with discharge planners and physicians.
• Develop relationships with referral partners and identify outreach opportunities.
