Job summary

Gentiva seeks an experienced Hospice Clinical Liaison to manage referrals, admissions, and education for patients and families. You will coordinate intake, consult with physicians and discharge planners, and serve as a liaison to ensure rapid, compassionate care delivery.

Strong hospice knowledge, patient assessment, and excellent communication are essential. Expect travel to hospitals and facilities and active outreach to partners.

Qualifications

• Licensed Nurse with three to five years of hospice experience required.
• BSN with five or more years of hospice experience preferred.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, and other professionals.
• Assess patient eligibility for hospice services by reviewing records and communicating with families.
• Educate patients and families on hospice care, processes, and expectations with compassion.
• Complete necessary documentation and coordinate intake and admission while meeting legal and payer requirements.
• Serve as a liaison between referral sources and the hospice care team for seamless handoffs.
• Make daily visits to hospitals and facilities to stay accessible to discharge planners and physicians.
• Build and maintain relationships with referral partners and identify new outreach opportunities.
