Qualifications

• Licensed RN in the state of employment.
• BSN preferred for hospice roles.
• Minimum 3 years of hospice experience required.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians and case managers.
• Assess patient eligibility for hospice by reviewing records and discussing with physicians and families.
• Educate patients and families on hospice care, processes, and benefits.
• Coordinate intake and admission ensuring legal, clinical and payer requirements are met.
• Serve as liaison between referral sources and hospice team for seamless handoffs.
• Make daily visits to hospitals and facilities to stay connected with discharge planners and physicians.
• Build and maintain relationships with referral partners and identify outreach opportunities.
