Job summary

Gentiva Hospice seeks an experienced Hospice Clinical Liaison to streamline intake and referrals, coordinate admissions, and strengthen relationships with hospitals, physicians, and case managers. As RN with hospice expertise, you will educate families about hospice, ensure documentation meets payer requirements, and accompany patients through the admission process with compassion and clarity.

This full-time role involves daily hospital and facility visits, building partnerships, and driving

Qualifications

• RN license with hospice experience required.
• CPR certification.
• Valid driver’s license and auto insurance.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals.
• Assess patient eligibility for hospice services by reviewing medical records and communicating with physicians/families.
• Educate patients and families on hospice care, how it works, and its value.
• Complete documentation and coordinate intake/admission, ensuring legal, clinical, and payer requirements are met.
• Serve as liaison between referral sources and hospice team for seamless handoffs.
• Make daily visits to hospitals and facilities to stay visible with discharge planners and physicians.
• Build and maintain relationships with referral partners and identify outreach opportunities.
