Job summary

Bon Secours Home Care and Hospice by Compassus is seeking a Hospice Team Coordinator to oversee documentation and data integrity for hospice patients. You will coordinate with the interdisciplinary team to ensure timely access to care and accurate billing through precise record-keeping.

The role requires detailed data entry, documentation of admissions, and effective communication with physicians, nurses, and families to support high-quality, compassionate care in a home-based setting.

Qualifications

• Experience handling medical records or medical office support preferred.
• Strong written and verbal communication with healthcare teams and families.
• Ability to manage multiple data streams and ensure accuracy.

Responsibilities

• Accurately enters and manages patient data including referral intake and demographics.
• Ensures admission docs are complete with CTI and order to admit.
• Uploads documents into EMR and HCHB systems for record-keeping.
• Monitors DME and medical supply orders and reports trends to management.
• Notifies providers of patient discharge and coordinates IDT activities.
• Acts as central contact for hospice team, patients, and families; handles calls and messages.
