Job summary

Bon Secours Home Care and Hospice by Compassus is seeking a Hospice Team Coordinator to model our values and coordinate delivery of hospice care. You will maintain patient records, manage admissions documents, and facilitate communication among the IDT, patients, and families, ensuring high-quality care and compliant documentation.

Responsibilities include data entry, EMR/HCHB uploads, billing alignment, and coordinating with physicians and therapy teams.

Qualifications

• High school diploma or GED required.
• Experience with medical records or medical office support preferred.
• Strong written and verbal communication skills.

Responsibilities

• Enter and manage patient data including referrals and demographics.
• Ensure admission documentation is complete with CTI and order to admit; upload into HCHB.
• Maintain patient records in HCHB and generate regulatory reports.
• Monitor DME orders and inform management of trends.
• Process physician orders and ensure timely returns.
• Verify and upload paper visit records into EMR and HCHB.
• Organize documents in patient charts for easy retrieval.
• Confirm care level matches billable level for accurate billing.
• Collect decertification paperwork and notify providers of discharge.
• Coordinate with IDT and act as main contact for patients and families.
• Handle calls, take messages, and manage the phone system.
• Assist DCS with IDT prep and updating plans of care; send UPOCs.
• Prepare patient binders in facilities.
• Perform other duties as assigned.
