Compassus in the United States is seeking a Hospice Team Coordinator to oversee the delivery of hospice care, ensure accurate documentation, and coordinate communication among patients, families, and the interdisciplinary team.

This role emphasizes adherence to regulatory standards, effective data management, and strong collaboration with physicians, nurses, and support staff to promote high-quality patient care.

Qualifications

• High school diploma or GED required; 2+ years in medical records or office support preferred.

Responsibilities

• Accurately enters and manages various patient data, including referral intake and demographics.
• Ensures admission documentation is complete with CTI and order to admit.
• Uploads documents into the HCHB system for record-keeping.
• Uses HCHB to maintain patient records and generate regulatory reports.
• Monitors DME and medical supplies orders and informs management of trends.
• Verifies and uploads paper-based visit records into EMR and HCHB.
• Organizes uploaded documents in patient charts for retrieval.
• Ensures care level matches billable level for accurate billing.
• Collects decertification paperwork and uploads it.
• Notifies providers of patient discharge through coordination notes.
• Initiates requests for medical records and hospital inpatient charts as needed.
• Facilitates coordination among the IDT.
• Acts as central point of contact for hospice team, patients, and families.
