Job summary

Providence at Home with Compassus is seeking a Hospice Team Coordinator to oversee patient data management, admissions documentation, and care coordination for the hospice team. The role emphasizes accurate record-keeping, regulatory compliance, and collaborative communication with physicians, nurses, and support staff.

The Coordinator will support the Hospice Director of Clinical Services, arrange agendas, and ensure timely updates to care plans, while serving as a primary contact for patients

Qualifications

• High school diploma or GED required.
• At least two years of experience with medical records, medical office, or business office support highly preferred.
• Strong written and verbal communication skills.

Responsibilities

• Accurately enters and manages patient data including referrals and demographics.
• Ensures admission documentation is complete and uploaded to the HCHB system.
• Uses HCHB to maintain patient records and generate compliance reports.
• Monitors DME and medical supplies orders and notifies management of trends.
• Coordinates care with the IDT and acts as central point of contact for patients and families.
