Job summary

Compassus is seeking a Hospice Team Coordinator to oversee delivery of hospice care, ensure accurate records, and promote compassionate, compliant patient support. You will coordinate data entry, documentation, and interdisciplinary team communication to ensure high-quality care.

You will manage admissions documentation, utilize HCHB and EMR systems, and serve as the main contact for patients and families. Prior medical records experience preferred and a commitment to Compassus values.

Qualifications

• High school diploma or GED required.
• 2+ years of experience with medical records, medical office, or business office support preferred.

Responsibilities

• Enter and manage patient data including referrals and demographics.
• Ensure admission docs complete with CTI and order to admit; upload to HCHB.
• Maintain patient records in HCHB and generate compliance reports.
• Coordinate IDT and communicate with patients and families.
• Handle phone calls and scheduling; transfer calls to appropriate personnel.
• Assist DCS with IDT activities and update plans of care.
• Prepare patient binders and perform other duties.
