Job summary

Compassus seeks a Hospice Team Coordinator to model our values and coordinate hospice care delivery with a focus on accuracy, communication, and collaboration. You will manage patient data, ensure complete admissions, and support the IDT to promote best outcomes for patients and families.

Key responsibilities include data entry in referrals and demographics, documentation uploads, and ensuring billing aligns with care levels while maintaining strong records. Strong communication is essential.

Qualifications

• High school diploma or GED required.
• At least two years of experience with medical records or medical office support preferred.
• Strong written and verbal communication skills.

Responsibilities

• Accurately enters and manages patient data, including referrals and demographics.
• Ensures admission docs are complete with CTI and order to admit, uploads to HCHB.
• Uses HCHB/EMR to maintain records and generate compliance reports.
• Coordinates IDT and serves as central point of contact for patients and families.
• Handles calls, messages, and maintains a welcoming atmosphere.
• Supports pre/post-IDT activities and prepares plans of care updates.
• Prepares patient binders and aids in billing alignment with level of care.
