Job summary

Compassus is seeking a Hospice Team Coordinator to oversee patient data, admissions, and care coordination in our home care and hospice programs. You will manage records in HCHB, ensure complete documentation, and support the interdisciplinary team to maintain high-quality patient care.

The role requires strong communication, attention to detail, and the ability to coordinate with physicians, nurses, and families.

Qualifications

• High school diploma or GED required.
• At least two years of experience with medical records, medical office, or business office support preferred.
• Strong written and verbal communication, and ability to coordinate with healthcare professionals.

Responsibilities

• Accurately enters and manages patient data, including referrals and demographics.
• Ensures admission documents are complete with CTI and order to admit.
• Uploads documents into the HCHB system and maintains patient records.
• Generates reports to demonstrate regulatory compliance and adds coordination notes.
• Monitors DME and medical supplies orders and informs management of trends.
• Coordinates among the IDT to ensure holistic patient care.
• Acts as central point of contact for hospice team, patients, and families.
• Handles calls, takes messages, and manages the phone system.
• Prepares agendas and UPOCs for IDT activities and sends to attending physicians.
• Prepares patient binders for facilities and performs other duties as assigned.
