Job summary

Providence at Home with Compassus is seeking a Hospice Team Coordinator to oversee delivery of hospice care, coordinate patient data, and maintain accurate records across HCHB and EMR systems.

The role emphasizes coordination with physicians, nurses, and the IDT, ensuring billing alignment and high-quality patient care. A strong emphasis on communication, confidentiality, and ethical conduct is required.

Qualifications

• High school diploma or GED required.
• At least two (2) years of experience with medical records, medical office, or business office support highly preferred.

Responsibilities

• Enter and manage patient data, including referrals and demographics.
• Ensure admission documentation is complete, signed, and dated with attending physician CTI.
• Upload documents into HCHB and EMR systems for complete records.
• Coordinate among the interdisciplinary team (IDT) to ensure holistic patient care.
• Assist with discharge notifications and requests for medical records as needed.
