This on-site role focuses on timely patient care, accurate documentation, and strong collaboration with physicians and leadership. Responsibilities include ensuring plan-of-care compliance, scheduling equitable caseloads, and driving performance improvements through QAPI initiatives.

Qualifications

• Deep understanding of hospice philosophy and end‑of‑life care.
• Proven ability to work within an Interdisciplinary Group (IDG).
• Proficient in EMR systems; Homecare Homebase preferred.
• Strong knowledge of Medicare/Medicaid and accrediting standards.
• Familiarity with QAPI and performance improvement initiatives.
• Ability to analyze operational and clinical data for improvement.
• Excellent communication with patients, families, staff, and physicians.
• Strong problem-solving and decision-making in fast-paced environments.

Responsibilities

• Coordinate daily operations of the assigned hospice team and ensure care aligns with the plan of care and regulatory standards.
• Monitor admissions, visits, and transitions to maintain speed to care and satisfaction.
• Oversee team scheduling for equitable caseloads and timely adjustments; escalate staffing concerns.
• Manage EMR workflow, documentation, and reporting; ensure accuracy and timeliness.
• Lead IDG meetings and coordinate with the Director of Clinical Operations; document outcomes.
• Collaborate with physicians for timely certifications and symptom management; ensure orders are uploaded.
• Coordinate transfers and travel arrangements for patients under travel contracts.
• Ensure Face-to-Face visits are scheduled promptly and documented properly in the EMR.
• Run EMR reports to identify bottlenecks and resolve scheduling and billing issues.
• Act as EMR workflow owner and escalate deficiencies to leadership.
• Provide EMR training and support; work with EMR team to improve workflows.
• Ensure compliance with Medicare Conditions of Participation and state standards.
• Support QAPI initiatives and performance improvement plans.
• Monitor caseload ratios and productivity with leadership; assist with corrective actions.
• Serve as a communication link between interdisciplinary team, physicians, facilities, and leadership.
• Escalate patient/family concerns and participate in service recovery.
• Foster accountability, collaboration, and professional growth within the team.
• Perform other duties as assigned.
