Job summary

ALIGN HOSPICE CARE LLC is seeking a Registered Nurse Case Manager to join our hospice team in Georgia. You will formulate the best care plan for each patient, ensure records are up to date, and maintain clear communication between the patient, family, and care team as the primary contact for care-related matters.

Qualifications

• Must be currently licensed as a Registered Nurse (RN) in the employing state.
• RN case management experience is preferred.
• Excellent interpersonal and client care skills.
• Valid driver’s license is required.
• First aid/CPR certification required.

Responsibilities

• Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans.
• Administer medications and monitor patients for side effects or reactions.
• Document changes in patient conditions and report on changes to the supervisor.
• Maintain accurate records of patient care and discuss observations with the supervisor.
