Three Oaks Hospice is seeking a Team Manager to lead an interdisciplinary hospice team in a non-remote, on-site capacity. You will own the EMR workflows, coordinate IDG meetings, and ensure timely patient visits and accurate documentation.

You will collaborate with leadership to maintain Medicare compliance, monitor performance metrics, and support staffing decisions to optimize patient care and family satisfaction.

Formación

• Experience coordinating interdisciplinary hospice teams.
• Proficient EMR usage with accurate documentation.
• Knowledge of Medicare/Medicaid and accreditation standards.
• Strong communication with patients, families, and staff.
• Ability to analyze data for performance improvement.

Responsabilidades

• Coordinate daily operations of the assigned hospice team adhering to the plan of care.
• Monitor admissions, visits, and transitions to ensure timely care.
• Oversee team scheduling and equitable caseloads with Intake/On-Call.
• Manage updates to the patient record and EMR documentation.
• Facilitate IDG meetings and document outcomes.
• Ensure regulatory compliance with Medicare Conditions of Participation.
• Escalate issues and support service recovery as needed.
