Three Oaks Hospice, Inc. is seeking a Director of Clinical Operations to provide strategic leadership and operational oversight across hospice teams.

Qualifications

• BSN is required with an active RN license.
• Minimum 2 years of clinical management or supervisory experience in hospice or home health.
• Strong knowledge of Medicare/Medicaid and accrediting standards; QAPI experience.

Responsibilities

• Direct the overall clinical operations of the assigned team(s) ensuring compliance and high-quality care.
• Supervise and support RNs, LPNs, CNAs, social workers, chaplains, and IDG members.
• Ensure policies and evidence-based practices are consistently implemented across teams.
• Lead admissions flow, certifications, and symptom management in collaboration with medical leadership.
• Lead IDG meetings and contribute to QA/QI programs and corrective actions.
• Monitor KPIs (CAHPS, audit scores, HOPE, ALOS) and manage staffing models and budgets.
• Mentor and develop staff for performance, retention, and succession planning.
• Coordinate with ED, SDCO, AVPO, RVPO and leadership on budgets and productivity.
