Job summary

Three Oaks Hospice is seeking a Director of Clinical Operations to provide strategic leadership, oversee clinical and non-clinical teams, and ensure high-quality hospice care across the agency while maintaining regulatory compliance. You will partner with medical directors and executive leaders to optimize staffing, drive performance improvements, and manage budgets.

Strong IDG leadership, QA/PIP experience, and EMR proficiency are essential.

Qualifications

• BSN required; RN license active.
• Minimum of 2 years of clinical management or supervisory experience in hospice or home health.
• Strong knowledge of hospice philosophy and end-of-life care.
• Experience leading an IDG and coordinating across departments.
• Proficient in EMR systems.

Responsibilities

• Direct the overall clinical operations of the assigned team(s) with regulatory compliance.
• Supervise RN, LPN/LVN, CNA, SW, Chaplains, TM, and IDG members.
• Ensure timely hospice admissions and care coordination.
• Lead IDG meetings and participate in QAPI and PIPs.
• Oversee regulatory readiness and accreditation surveys.
• Monitor KPIs and manage budgets and staffing models.
• Foster accountability and staff development.
