Job summary

Infinity Hospice Care LLC seeks a Patient Care Advocate to grow hospice referrals across the assigned territory. You will develop partnerships with physicians, facilities, and care-at-home providers to expand services and improve patient access.

This role requires proactive sales calls, presentations, and quarterly territory planning. You will maintain relationships with referral sources, document activity, and stay current on programs, regulatory requirements, and provider needs.

Qualifications

• Bachelor’s degree in marketing or related field or equivalent professional experience.
• Minimum of two (2) years of experience in hospice sales or a related industry with a proven record of growth.
• Proficiency in Microsoft programs for communications, presentations, tracking, and spreadsheets.
• CRM experience preferred.

Responsibilities

• Develops territory plans to achieve growth goals and executes agreed strategies.
• Makes effective sales calls and presentations to targeted referral sources.
• Builds relationships with physicians, facilities, care-at-home providers, and community agencies.
• Travels locally to promote hospice services and educate the healthcare community.
