Job summary

Affinity Health Management is seeking a hospice aide to work under an RN, delivering patient-centered care per the plan of care. You will report observations, promote comfort, and support families while maintaining safety and quality of care.

The role emphasizes documentation, confidentiality, and teamwork across facilities, with responsibilities ranging from ADL support to mobility assistance and medication reminders in a licensed setting.

Qualifications

• Completion of state approved CNA course or equivalent training.
• Current Healthcare Provider CPR certification.
• Covid vaccination as per facility policy.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and terminal care.
• Prior home health/hospice experience preferred.
• Basic computer skills for EMR documentation.
• At least 18 years old.
• Valid driver’s license and insured vehicle.

Responsibilities

• Perform personal care tasks per patient assignment and report gaps to the Case Manager.
• Provide patient and family with non-intrusive, confidential communication.
• Meet safety needs and use equipment safely.
• Assist with baths, back rubs, oral hygiene, shampoos, and bed linen changes.
• Assist in dressing and undressing as assigned.
• Assist feeding; avoid homemade foods.
• Monitor skin to prevent tissue breakdown.
• Report pain or condition changes to RN Case Manager and note caregiver issues.
• Assist in ambulation and prescribed exercises.
• Provide ROM and transfers as directed.
• Assist with self-administered medications as allowed by state regulations.
• Assist with bedpans/urinals, light laundry, and waste disposal.
• Perform ROM and simple procedures with competency.
• Provide respite for patient/family when on-site.
• Keep living area clean and orderly.
• Follow care procedures and professional conduct standards.
• Participate in QA/QI activities.
• Respect patient/family environment and needs.
• Report non-compliance to health authorities as required.
• Report suspected abuse/neglect to proper authorities.
