Job summary

Affinity Hospice Management LLC is seeking an experienced Hospice Aide in Ohio to support patient comfort under the supervision of a registered nurse. You will perform personal care tasks, assist with daily living activities, and document observations to ensure quality, continuity, and safety of care.

You will work with patients and families using respectful communication, maintain confidentiality, and follow the plan of care.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training
• Current Healthcare Provider CPR certification (hands-on verification required)
• Covid vaccination per facility guidelines
• High school diploma or GED
• Ability to read and follow written instructions and document care
• Understands hospice philosophy; comfortable with terminal illness care
• 1 year hospice/home health/long-term care experience preferred
• Basic computer skills to document in EMR
• Must be at least 18 years old
• Licensed driver with insured vehicle in good working order

Responsibilities

• Perform personal care tasks after assignment and promptly attend to requests.
• Provide patient and family with respectful communication and confidentiality.
• Ensure safety and proper use of equipment (foot stools, side rails, O2).
• Assist with bathing, hygiene, shampoos, and changing bed linens.
• Assist in dressing and undressing as assigned.
• Assist in feeding; avoid homemade foods prepared by staff.
• Monitor skin condition to prevent tissue breakdown and report concerns.
• Report pain or changes in condition to RN Case Manager promptly.
• Assist in ambulation, transfers, and range of motion as directed.
• Assist with self-administered medications as allowed by state law.
• Assist with bedpans, urinals, light laundry, and housekeeping tasks.
• Document care and observations accurately in EMR.
