AccentCare is seeking an experienced RN to join our hospice after-hours admissions team in Florida. This full-time role involves on-call duties, admissions, and care transitions, with a focus on coordinating interdisciplinary care.

Qualifications

• Current RN licensure in the applicable state is required, with an associate degree preferred.
• CPR certification is recommended (required for direct patient care in Georgia).
• Must demonstrate willingness to participate in an on-call rotation and maintain reliable transportation, valid driver's license, and state-required liability auto insurance.
• Strong communication and organizational skills are essential, with proficiency in Microsoft Office Suite and EMR systems.
• Ability to collaborate effectively within an interdisciplinary team.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with patients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evaluate individualized plans of care in alignment with regulatory requirements.
• Maintain timely and accurate documentation, ensure care plans reflect current patient status, and coordinate care with the team to optimize resource use.
• Provide education and emotional support to patients and caregivers; participate in on-call rotations, admissions, and transitions as directed.
• If serving as Care Manager, coordinate all aspects of patient care including scheduling, med management, plan of care oversight, IDG collaboration, and supervision of staff.
• If serving in On-Call capacity, respond to triage calls, perform visits, coordinate care with Administrator On Call, and participate in IDG meetings.
