Job summary

AccentCare is seeking an After-Hours Admission RN to provide skilled nursing care during after-hours hours. The role includes participating in on-call rotations, admissions, and care transitions to support patients and families in a hospice setting.

The position requires current RN licensure in the applicable state, an associate degree preferred, and strong communication and organizational skills. Proficiency with EMR systems and Microsoft Office is expected.

Qualifications

• Current licensure as a Registered Nurse in the applicable state is required, with an associate degree preferred.
• Must participate in an on-call rotation and have reliable transportation and valid driver’s license.
• Strong communication and organizational skills are essential, with proficiency in Microsoft Office Suite and EMR systems.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with patients and care team to develop and evaluate care plans.
• Maintain timely and accurate documentation and coordinate resources for optimal patient care.
• Provide education and emotional support to patients and caregivers and participate in on-call rotations, admissions and care transitions.
