Qualifications

• Licensed as a Registered Nurse in the state(s) of practice; associate degree preferred and CPR certification recommended (required for direct patient care in Georgia).
• Must meet state-specific regulatory requirements.
• Previous hospice/home health or related clinical experience desired.

Responsibilities

• Assess patient and family needs across physical, psychological, social and spiritual domains; collaborate with IDG to plan and evaluate care.
• Initiate and maintain communication with attending physician, Medical Director, IDG and other agencies to coordinate care.
• Coordinate admissions processes and resource use for patient and family.
• Document per protocol and keep patient records up to date; reflect care plan progress.
• Participate in On-Call or Admissions rotation as directed.
• Educate patient and caregiver(s) based on knowledge needs and plan of care.
