AccentCare, Inc. in Miami seeks an After Hours Registered Nurse / RN, Hospice for a full-time, on-site role covering Dade County. North Miami residents are preferred.

The RN will perform assessments, coordinate plans of care, and support patients and families with compassionate hospice services. Qualifications include an active RN license in the state, hospice/home health experience, and willingness to participate in on-call rotations.

Qualifications

• Current RN licensure in the applicable state.
• Associate degree preferred; BSN a plus.
• CPR certification recommended for direct patient care.
• Experience in hospice, home health, medical-surgical, geriatrics or pediatrics.
• Willingness to participate in on-call rotation and reliable transportation.
• Valid driver’s license and state-required auto liability insurance.
• Strong communication and organizational skills; teamwork in interdisciplinary groups.

Responsibilities

• Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the IDG to develop and evaluate care plans.
• Maintains timely documentation and ensures care plans reflect patient status; communicates with the care team.
• Provides education and emotional support to patients and caregivers; participates in on-call rotations and care transitions.
• If Care Manager, coordinates scheduling, medication management and supervision of staff; participates in IDG meetings.
• In on-call capacity, responds to triage calls, performs visits and coordinates care with leadership; participates in training and committees.
