Job summary

AccentCare Inc is seeking an experienced RN for after-hours hospice admissions in the Baltimore area. This full-time, on-call role requires an active RN license, ADN preferred, and willingness to participate in rotations with reliable transportation and CPR-competent care.

Qualifications

• Must hold current RN license in the applicable state; ADN preferred.
• CPR certification recommended for direct patient care.
• Willingness to participate in on-call rotation and maintain reliable transportation and license.
• Strong communication, organization, and collaboration within an interdisciplinary team.

Responsibilities

• Conduct comprehensive nursing assessments and coordinate care plans with patients, families, and IDG.
• Maintain timely documentation and align care plans with patient status and regulatory requirements.
• Provide education and emotional support to patients and caregivers; participate in on-call and care transitions.
• Coordinate care if acting as Care Manager, including scheduling, meds, plan oversight, IDG collaboration.
• Respond to triage calls and support on-call visits and patient conferences.
