Job summary

AccentCare is hiring a Hospice Registered Nurse for night shifts (Fri, Sat, Sun) in Clearwater, FL. You will provide compassionate, patient-centered care, perform assessments, and collaborate with the IDG to coordinate plans of care. This full-time role includes on-call responsibilities and mileage reimbursement.

Strong communication and teamwork are essential, with a focus on aging-in-place care. The ideal candidate holds an active RN license, preferably an associate degree, and has experience

Qualifications

• RN licensure in the applicable state required, associate degree preferred.
• CPR certification recommended (required for direct patient care in Georgia).
• Experience in hospice, home health, medical-surgical, geriatrics, or pediatrics preferred.
• Willingness to participate in on-call rotation and maintain reliable transportation.

Responsibilities

• Perform comprehensive nursing assessments and develop care plans with IDG.
• Document timely and accurately; coordinate care and resource use.
• Educate and support patients and caregivers; participate in on-call rotations.
• Coordinate all aspects of patient care as Care Manager if applicable.
• Respond to triage calls and support care transitions when on-call.
