Transitions Care is seeking a compassionate Registered Nurse for Hospice After Hours to provide expert clinical care and triage to patients and families during evenings, nights, and weekends.

You will perform symptom assessment and management, respond to urgent needs, make home or facility visits, and coordinate with the interdisciplinary team to ensure continuity of care while educating and supporting caregivers.

Qualifications

• Licensed RN with hospice or end-of-life care experience.
• Ability to work evenings, nights, and weekends.
• Strong assessment, documentation, and communication skills.

Responsabilités

• Provide symptom assessment and management for hospice patients.
• Perform triage and respond to urgent needs during after-hours.
• Make home or facility visits as needed.
• Coordinate with interdisciplinary team to ensure continuity of care.
• Educate and support caregivers and families.
• Document thoroughly and maintain accurate records.
• Work autonomously within a collaborative, values-driven organization.
