Job summary

Elite Hospice, part of LHC Group and within the Optum family, seeks a Per Diem After hours Registered Nurse to deliver comprehensive hospice care after regular hours. This role focuses on triage, crisis response, and after-hours visits to assess and support patients and families.

Responsibilities include timely nursing evaluations, documentation, and ensuring care remains patient-centered, with a strong emphasis on safety and quality in after-hours care delivery.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current CPR certification.
• Minimum one year of clinical experience.
• Current driver's license and access to a dependable vehicle or public transportation.

Responsibilities

• Triages patient needs over the phone until an in-person visit by a nurse can occur.
• Responds urgently to crisis calls after hours.
• Performs initial nursing evaluation within 48 hours of referral for hospice eligibility.
• Conducts after-hours visits including admissions, recertifications and routine visits.
• Identifies patient/family needs and reassesses at least every 15 days.
• Documents problems, goals, interventions and responses to care.
