Arc Hospice & Palliative Care is hiring a Hospice RN – After Hours to provide compassionate clinical care evenings, overnights, weekends and holidays. You will respond to urgent needs, manage symptoms, support families and facilitate safe transitions of care in patient homes, facilities, hospitals, and emergency departments.

The role emphasizes independent decision-making, regulatory knowledge, and timely documentation in the EMR, with on-call coverage for a defined service area.

Formación

• Graduate of an accredited nursing program; Associate’s degree or higher preferred.
• Current RN license in the state of practice or multi-state license.
• Minimum of one year hospice, palliative, or home health experience preferred.
• Strong clinical judgment, triage, and documentation skills.
• Ability to work independently and make sound decisions in urgent situations.
• Knowledge of hospice regulations and Medicare Conditions of Participation.
• CPR certification and valid driver’s license required.

Responsabilidades

• Respond to calls and conduct in-home or facility visits as needed.
• Assess symptoms and provide interventions to promote comfort and safety.
• Coordinate with on-call providers for orders and care planning.
• Handle after-hours admissions and support families during transitions.
• Collaborate with ED and facility staff for hospice-appropriate discharges.
• Conduct admission visits to initiate hospice services after hours.
• Ensure accurate handoffs and real-time EMR documentation.
• Maintain on-call availability and regulatory/compliance documentation.
