Job summary

Compassus is seeking an Admissions Coordinator to process referrals, coordinate patient admissions, and enter admission data after all hospice requirements are met. The role involves supporting the program area, attending meetings, and communicating needs to supervisors.

Responsibilities include contacting physicians for orders, insurance pre-certification, scheduling visits, and guiding families through hospice options.

Qualifications

• Bachelor's degree preferred; 2–3 years related experience strongly preferred.
• Experience communicating with physicians, nurses, and other healthcare personnel is highly preferred.

Responsibilities

• Support designated program area and/or region.
• Attend and participate in staff meetings, educational programs and community events.
• Communicate identified needs and potential solutions to supervisor.
• Receive and process referrals from physicians, patients, families, friends, and community.
• Contact physicians to secure an admission order; ensure nurse presence or script for verbal orders if needed.
• Coordinate pre-certification with insurance providers.
• Schedule appointments for informed consents and RN assessments.
• Explain hospice services and benefits to patients and families.
• Notify team members of appointment times.
• Complete admission processes per policy and ensure chart is updated and signed.
• Market referral sources and promote hospice services to LTC/hospital staff.
