Job summary

Mercy Health Home Care and Hospice by Compassus seeks a Hospice Admissions Coordinator to process referrals, coordinate admissions, and enter patient data. This is a non-remote role requiring local residency, with a Monday–Friday schedule and 8am–5pm hours.

LPN preferred and prior hospice experience valued; strong communication with physicians and teams is essential. The role includes coordinating with IDT, marketing to referral sources, and ensuring complete documentation.

Qualifications

• High school diploma or GED required; bachelor's degree preferred.
• Two to three years of related experience strongly preferred.
• Experience communicating with physicians, nurses and healthcare personnel highly preferred.
• Experience in insurance/Medicare-Medicaid billing and reimbursement preferred.
• LPN preferred where applicable.

Responsibilities

• Receives and processes referrals from physicians, patients, families and community.
• Contacts physicians to secure an admission order; ensure nurse presence if needed.
• Schedules informed consent and RN assessment visits.
• Explains hospice services and benefits to patients and families.
• Completes admission process and notifies the IDT and providers.
• Enters admission data into a third-party system and maintains charts.
• Markets hospice services to referral sources and community partners.
