Job summary

Affinity Health Management seeks an Admissions Nurse to join the hospice team in Dayton, OH. You will perform initial comprehensive assessments, plan and coordinate care, and provide skilled nursing interventions to ensure patient comfort in line with hospice policies.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice in Ohio.
• 1 year of nursing experience in the last 3 years, or a BSN with 2 years’ experience in public health, home care, or hospice.
• Offer contingent upon references and criminal background checks.
• Excellent observation, communication, problem solving, and basic math skills.

Responsibilities

• Complete initial comprehensive assessment of patient and family to determine hospice needs.
• Provide skilled nursing care and utilize the nursing process.
• Develop and revise the plan of care based on patient needs and status.
• Coordinate patient care as Case Manager when assigned.
• Communicate with physicians and care team; document all notes and changes.
• Assist with activities of daily living and patient/family education.
