Job summary

Affinity Hospice Management LLC seeks an Admissions Nurse to join the interdisciplinary team. You will perform skilled nursing assessments, plan care at admission, and coordinate with physicians, case managers, and families to ensure comfort and appropriate symptom control.

This on-call role emphasizes collaboration and rapid care planning. Responsibilities include initiating and revising care plans, educating patients and caregivers, and maintaining accurate clinical notes while ensuring safety

Qualifications

• Graduate of an accredited school of nursing.
• Virginia RN license in good standing.
• Minimum 1 year nursing experience in the last 3 years; hospice/public health preferred.
• Contingent upon satisfactory references and criminal background checks.
• Excellent observation, communication, problem solving, and math skills; nursing skills per competency checklist.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Perform initial comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Assess and evaluate patient status; plan, revise, and coordinate care.
• Initiate and adjust care plans; determine nursing diagnoses.
• Administer medications and treatments per physician's plan.
• Counsel patient and family on nursing and related needs.
• Provide health instructions and support activities of daily living.
• Act as Case Manager when assigned; coordinate patient care with team.
• Complete notes and communicate changes in patient condition to the physician and team.
• Participate in on-call duties and ensure equipment and services are arranged.
