You will review referrals from multiple sources via phone, fax, and electronic systems, and coordinate initial hospice assessments with the clinical team.

The role requires an active RN license, at least 1 year of RN experience (hospice/admissions preferred), strong organizational and communication skills, and EMR proficiency.

Qualificações

• Active RN license in the applicable state is required.
• Minimum 1 year of RN experience, preferably in hospice, home health, or admissions/intake.
• Strong clinical judgment and ability to manage multiple priorities in a fast-paced setting.
• Proficiency with EMR systems and healthcare documentation.

Responsabilidades

• Receive, review, and prioritize hospice referrals to support timely admissions.
• Respond to referral inquiries via phone, fax, or electronic systems.
• Enter and maintain patient demographics, clinical, and insurance data in the EMR.
• Coordinate admission scheduling and communication with clinical teams.
• Review and co-sign referrals as required; collaborate with billing and verification partners.
• Apply Medicare, state, and hospice regulatory guidelines when processing referrals.
