Job summary

Mercy Health Home Care and Hospice by Compassus is seeking an Admissions Coordinator to process referrals, coordinate patient admissions, and enter data after hospice requirements are met.

RN preferred; other licensed clinicians with related experience will be considered. The role involves communicating with physicians, family members, and care teams, and may require pre-certification steps with insurers.

Qualifications

• RN preferred.
• Two to three years of related experience and/or training strongly preferred.
• High school diploma or GED required.
• Bachelor's degree preferred.
• Other clinical licensure strongly considered.
• Experience communicating with physicians, nurses and other healthcare personnel highly preferred.
• Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.

Responsibilities

• Process patient referrals and coordinate admissions.
• Enter admission data into systems.
• Read charts and determine needed documentation for certification.
• Attend staff meetings and educational programs.
• Explain hospice services and benefits to patients and families.
• Notify Interdisciplinary Team of appointment times and changes.
• Market referral sources and promote hospice services.
• Provide back-up support for Hospice Care Consultant.
