Job summary

Providence at Home with Compassus in Beaverton, OR is seeking a Hospice Admissions Coordinator to manage referrals, coordinate admissions, and enter patient admission data in our system. You will work with physicians, families, and the care team to ensure smooth intake and proper documentation.

Responsibilities include processing referrals, obtaining orders and authorizations, scheduling assessments, explaining hospice services, and communicating with the IDT.

Qualifications

• High school diploma or GED required.
• Bachelor's degree preferred.
• Two to three years of related experience and/or training strongly preferred.
• Experience communicating with physicians, nurses and other healthcare personnel highly preferred.
• Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.

Responsibilities

• Process patient referrals and coordinate admissions.
• Attend staff meetings, educational programs and community events as requested.
• Communicate identified needs and potential solutions to supervisor.
• Receive and process referrals from physicians, patients, families, and community.
• Contact physicians to secure an order for admission and ensure nurse presence if needed.
• Coordinate pre-certification authorization with insurance companies.
