Job summary

Providence at Home with Compassus is seeking an Admissions RN to coordinate and process hospice referrals, ensuring timely and seamless patient admissions. The role serves as a clinical resource for intake, reviewing referrals from multiple sources via phone, fax, and electronic systems, and coordinating with physicians and the clinical team.

This full-time position requires RN licensure, at least 1 year of RN experience, and preferred hospice/home health admissions experience.

Qualifications

• Active RN license in applicable state required.
• Minimum 1 year of RN experience required.
• Hospice, home health, or admissions/intake experience preferred.
• BLS/CPR certification required within 30 days of hire.
• Strong clinical judgment and organizational skills.

Responsibilities

• Receive, review, and prioritize hospice referrals to support timely admissions and continuity of care.
• Respond promptly and professionally to referral inquiries and intake calls.
• Enter and maintain accurate patient demographic, clinical, and insurance information within the EMR.
• Obtain physician orders, document Certification of Terminal Illness (CTI) information, and ensure referral documentation is complete and compliant.
• Coordinate admission scheduling and communication with the clinical team, Patient Care Supervisors, and Patient Care Coordinators.
• Review and co-sign referrals entered by LVNs and non-clinical intake staff as required.
• Collaborate with billing, insurance verification, pharmacy, and PBM partners to facilitate smooth patient onboarding.
• Apply Medicare, state, and hospice regulatory guidelines when reviewing and processing referrals.
• Maintain accurate, timely documentation and support efficient workflow within the intake and admissions process.
• Perform additional duties as assigned.
